Notice to Our Patients and Families

We would like to inform our patients and families that, after careful consideration, Milestones Pediatric will be permanently closing effective April 3rd, 2026. It has been a privilege to care for your children and to be part of your families’ lives.

To support continuity of care, we will gladly provide copies of your child’s medical records. Medical records are confidential; therefore, you will need to complete a medical record release authorization form to allow records to be sent to the pediatrician of your choice free of charge. We encourage you to access the MyChart Patient Portal to download all medical records. You can access the Patient Portal or download the Medical Release Form by visiting our website:

Our phone (469) 678-8204 and fax (469) 625-2883 will remain active through April 3rd, 2026. After April 3rd, 2026, all patient records will be transferred to a secure, HIPAA-compliant medical records custodian, Desert River Solutions. After our clinic closure date, if you need your child’s medical records, please submit your request directly to Desert River Solutions. To request records, please visit the website below to sign and return the medical release form. All instructions are available on their webpage.
